Patron complaints or concerns in regard to library materials and/or programs must complete a "Request for Materials Removal" or "Request for Program Reconsideration" form. The form must be filled out in its entirety. Any complaints or concerns about a library material or program sent electronically will not be considered. Please return the form to the Malvern Public Library circulation desk during regular business hours, or mail the form/s to:
